Healthy Hearts & Plates is a free, community-led project supporting heart, kidney and diabetes health for residents in Waltham Forest.
The project combines gentle movement, culturally relevant food demonstrations and simple,
jargon-free health information to support prevention, early awareness and self-management of
long-term health conditions.
Can’t attend in person? You can still take part
If you’re unable to attend our in-person sessions, you can still be involved by completing our short online health questionnaire.
The questionnaire helps us understand what information and support would be most useful for you. It is not a medical assessment and does not replace advice from your GP or nurse.
Takes 5–7 minutes • Free • Confidential • Follow-up
If you choose to leave your contact details, a member of the Skills Within team will check in with you to share relevant resources, local support or information about future sessions.
